Participant Information & Liability Waiver

I, the undersigned, being aware of my own health and physical condition, and having knowledge that participation in any exercise or fitness program may pose risks to my health, voluntarily choose to participate in activities at G1 Elite Fitness and Performance LLC.

Having such knowledge, I hereby acknowledge and release G1 Elite Fitness and Performance LLC, its owners, trainers, representatives, agents, and successors from any liability for accidental injury, illness, or other issues which may result from participation in said activities. I fully assume all risks connected with participation and consent to engage in the programs offered.

I agree to disclose any physical limitations, disabilities, ailments, or impairments which may affect my ability to safely participate in fitness activities.
